The Mind Swarm is a sick abomination of the more common illithid. It is made up of extremely tiny single tentacles, looking almost like worms, that enjoy nothing more than extracting brains. Upon extracting the brains, they eat it and reproduce quickly, making the swarm even larger.

In combat situations, Mind Swarm tries to surround a creature, land on it, and extract its brain.
Distraction: Any living creature that begins its turn with a swarm in its space must succeed on a DC 14 Fort save or be nauseated for 1 round. The save DC is Con based.
Attach: In order to attach to a creature, a Mind Swarm must make a melee touch attack using its dexterity modifier + its base attack bonus. If it succeeds, it may extract next round. During this time, however, the affected creature may make a reflex save DC 18 to shake the swarm off. The save DC is dex based.
Extract: If a Mind Swarm begins its turn attached to a creature, it may extract its brains. Doing so kills the creature, unless it has more than one head. 1d4+1 rounds later, the swarm gains back half of its lost hitpoints and takes up 2 more squares than before as the brain is devoured. A creature with no brain is immune to this effect.
